SelectCare Health is a comprehensive outpatient rehabilitation facility that specializes in Pulmonary, Physical, and Occupational Therapy. Our rehabilitation facility is established and operated exclusively for the purpose of providing you the convenience of receiving the rehabilitation you need at one facility. SelectCare Health personnel are highly trained and will develop a treatment plan in conjunction with our facility medical director and your physician. We will use that treatment plan to ensure that you are progressing toward your goals and to ensure improvement in your physical or chronic condition.
SelectCare Health has a multi-disciplinary healthcare team which includes: a medical director, clinical administrator, registered nurse, physical therapists, respiratory therapists, occupational therapists and social workers. We have a multi-disciplinary team to make sure all of your injuries and illnesses are properly cared in one convenient facility. Our skilled professionals have the expertise to diagnose and treat any illness or injury. We use state of the art medical technologies to diagnose patients during their initial evaluation and to create a comprehensive treatment plan. Our team constantly evaluates and modifies the treatment plan to ensure patient satisfaction and a prompt and full recovery. All of our healthcare personnel are carefully screened to meet our high employment standards and ensure the highest levels of patient care.
Our facilities are conveniently located throughout Chicago and have top of the line medical and rehabilitation equipment for our patients. Our facilities have dedicated parking near the front door for all of our patients. We also offer complementary transportation for individuals who cannot drive or get a ride to our facility.
