PATIENTS

What if your loved one could have the same Registered Nurse (RN) or Certified Nurse Assistant (CNA) caring for her in the hospital and at home? If she needed Physical Therapy or Pulmonary Rehabilitation, wouldn't it be nice if you only had to make one phone call? At SCH, we offer a variety of services in the hospital, at home, or in an outpatient rehabilitation setting.

We understand and fully appreciate that finding the right person and organization to care for your loved one is oftentimes the hardest of decisions. You arduously ask yourself, your family members, and your friends for advice. It is hard enough having your loved one stay alone at a hospital for even one night. What about when he is discharged from the hospital and still requires care? Whom do you trust? Where do you turn? There are many grueling decisions when these situations arise, and fortunately SCH understands.

Our healthcare professionals (usually a CNA or Healthcare Aid, or at your request a Nurse) will stay in the hospital with your loved at his or her bedside. As you are probably aware, hospitals are notoriously short-staffed and do not have the means to provide continuous one-on-one service. That is where SCH fills the gap. Through our private care program, we provide healthcare professionals in the hospital or at home when the hospital is unable to provide someone. Remember, hospitals have limited resources to attend to multiple patients, and you might only feel comfortable with complete care for your loved one where someone stays in the room at all times. That is when you contact SCH. And as an added bonus, that same person can be with your loved one in your home when he/she is discharged from the hospital. We believe that continuity of care is a necessity and should be accessible to everyone.

We also offer a variety of clinical services in an outpatient setting such as Pulmonary Rehabilitation, Physical Therapy, and Occupational Therapy. Nobody wants to see a loved one held down by disease, injury, or illness. They deserve best clinical care from one company with people they can trust.

Our outpatient rehabilitation partners such as pulmonologists, orthopedic surgeons, cardiologists, and internists can attest to the superior patient care provided at our outpatient rehabilitation clinics. We understand that doctors, as well as patients, want to minimize recovery time while ensuring quality care. Physicians' offices should not be worried about faxing orders and making phone calls. SCH, through the use of technology, streamlines the entire referral process.
